PEEKUP PASSENGER PERSONAL ACCIDENT INSURANCE TERMS AND CONDITIONS

Underwritten by PGA SOMPO INSURANCE CORPORATION

The PeekUp Passenger Personal Accident Insurance is provided to passengers riding with PeekUp and underwritten by PGA SOMPO INSURANCE CORPORATION (“PGA SOMPO”).

This protection offers financial assistance in the event of accidental injury, disability, death, or trip disruption occurring during an active PeekUp ride.

Use of the service and/or purchase of an insured trip signifies acknowledgment and full acceptance of the coverage terms, conditions, limitations, and exclusions outlined herein.

I. ELIGIBILITY

Coverage applies exclusively to individuals riding as PeekUp passengers in a properly registered and accredited PeekUp vehicle at the time of the incident.

II. SCOPE OF COVERAGE

1. Accidental Death & Disablement

Provides compensation of up to ₱1,500,000 in the event of accidental death, permanent disablement, or loss of limb resulting from a sudden, external, violent, and visible event occurring during a PeekUp ride.

2. Trip Delay Benefit

Pays ₱100 if passenger pick-up is delayed by more than 15 minutes due to mechanical breakdown, accident, or other unforeseen circumstances beyond the driver’s control, including delays arising from unexpected personal circumstances on the driver’s end.

III. WHEN COVERAGE APPLIES

Coverage is active while the insured passenger is:

1. Boarding a PeekUp vehicle

2. In transit during the entire duration of the ride (whether moving or stationary)

3. For up to five (5) minutes after disembarkation and within one (1) meter from the vehicle

Coverage applies nationwide, at all hours, but only within the defined active trip window.

IV. LIMIT OF LIABILITY

Benefit payment shall not exceed the stated maximum coverage limits.

Medical-related claims shall be reimbursed based on actual expenses supported by official documents and within the applicable maximum limit.

V. NUMBER OF CLAIMS

A passenger shall be entitled to one (1) claim only, subject to the maximum benefit set forth in the Schedule of Benefits.Upon payment of such benefit, the passenger shall not be entitled to any further claims, and all coverage under this Policy shall automatically terminate.

VI. GENERAL EXCLUSIONS

No benefit shall be payable under this Certificate of Cover for any claim, loss, damage, expense, injury, disablement, or death

resulting from, arising out of, or in any way related—directly or indirectly—to any of the following:

A. Communicable Diseases

1. Illness, infection, or disease not caused solely by a covered accident.

2. Claims related to epidemics, pandemics, viral outbreaks, quarantine, movement restriction, or government-imposed lockdown.

3. Costs related to detection, monitoring, sanitizing, and containment of communicable disease.

B. Medical, Surgical and Health-Related Conditions

1. Pregnancy, miscarriage, or complications thereof.

2. Non-accident-related medical or surgical procedures.

C. Intentional or Criminal Acts

1. Suicide, attempted suicide, or self-inflicted injury.

2. Assault, murder, or attempted violence.

3. Commission of a criminal act, violation of traffic or road regulations, evading lawful apprehension, or conduct deemed unlawful whether civil or criminal in nature.

D. War, Terrorism, and Government Action

1. War, invasion, rebellion, civil unrest, or insurrection.

2. Acts of terrorism or threats thereof.

3. Seizure, confiscation, quarantine, or destruction of property by order of any authority.

4. Any claim where payment or coverage would violate domestic or international sanctions.

E. Nuclear and Radioactive Exposure

Loss or damage arising from nuclear reaction, radiation, radioactive contamination, or exposure to nuclear material or facilities.

F. Hazardous or High-Risk Activities

Injury sustained while mountaineering requiring ropes or guides, scuba diving with compressed air tanks, professional or amateur racing, skydiving, hang-gliding, or other extreme activities.

G. Alcohol or Prohibited Substance Use

Claims arising from accidents occurring while the insured is under the influence of prohibited drugs or intoxicating alcohol.

H. Consequential and Indirect Losses

Economic or indirect losses including loss of income, quarantine restrictions, travel disruption, or any circumstance not resulting from a covered accidental event within the valid coverage period.

VII. CLAIM DOCUMENT REQUIREMENTS

A. General Requirements

• Confirmation of insurance coverage

• Government-issued ID of the insured

• Government-issued ID of the beneficiary/claimant

• Police or incident report, or notarized affidavit

• Additional documentation that may be required for verification

B. Accidental Death Claim Requirements

• Official death certificate

• Medical or autopsy report, if applicable

• NSO Birth Certificate (if beneficiary is parent/child/sibling)

• NSO Marriage Certificate (if beneficiary is spouse)

VIII. CLAIMS PROCEDURE

1. Submit the completed Claim Form via the PGA Sompo Claims Portal.

2. Attach all required supporting documents.

3. Await assessment and validation. Additional requirements may be requested if necessary.

4. Upon approval, benefits will be released to the claimant.

Standard claims processing is five (5) to seven (7) business days upon submission of complete documentation. Request for additional documents does not constitute approval or admission of liability.

IX. CLAIMS AND PAYMENTS

1. CompensationCompensation shall be payable only upon full proof and verification of the claim.

2. Beneficiary / Payable ToLoss of life is payable to the beneficiary, or if none, to the estate of the Insured. All other indemnities are payable to the Insured.

3. Notice of Claimnotice of claim must be given within thirty (30) days of the accident, or as soon thereafter as reasonably possible.

4. Proof and EvidenceProof of loss and supporting evidence shall be furnished at the Insured’s expense. Medical or post-mortem examinations may be required at the Company’s expense.

5. Changes Affecting CoverageFailure to notify the Company of any changes in address, occupation, other insurance,

or health conditions may affect coverage.

X. POLICY CONDITIONS

1. Entire Contract / Changes – This Policy, including endorsements and applications, constitutes the entire contract. Any changes must be endorsed by an authorized Company official.

2. RenewalThe Policy may be renewed unless the Company provides at least forty-five (45) days’ prior notice of non-renewal or changes to coverage.

3. CancellationThe Policy may be cancelled in accordance with the provisions of the Insurance Code, with premiums refunded on a short-rate basis if requested by the Insured.

4. AssignmentNo assignment of benefits or change of beneficiary shall bind the Company unless formally endorsed by the Company.

5. Inflation ClauseArticles 1250 of the Civil Code (Philippines) shall not apply; liability is based on the Policy terms, not currency inflation.

6. Legal ActionAny action or suit must be initiated within twelve (12) months from notice of claim rejection.

7. Claims SettlementPayments shall be made within thirty (30) days after proof of loss and ascertainment. If ascertainment is delayed beyond sixty (60) days, payment shall be made within ninety (90) days of proof of loss.

8. Premium Payment The Policy shall be valid only upon full payment of the premium.

9. Claim FormsThe Company shall provide claim forms. If not received within fifteen (15) days, written proof of lossshall suffice.

10. Proof of Loss Completed claim forms and proof of loss must be furnished within ninety (90) days from the date of loss. Late submissions are accepted if reasonably justified.
